A hospital has finally admitted that it failed a baby boy by making ‘catastrophic mistakes’ – more than seven years after the child was shaken to death. Kyle Keen was killed by his stepfather in 2006 after NHS staff failed to report his ‘suspicious bruising’ to police or social services.
